Welcome to the Healthy Lifestyle Challenge 2011. We hope that during the next ten weeks you will develop at least one new healthy habit. Even beyond that, our hope is that you will have fun and positively improve the culture of wellness in your home, workplace and community. We are going to do everything possible to keep you engaged and committed for the next ten weeks.
Step 1: Find your baseline: This is for your own personal use. We will ask you at the end of 10 weeks to recalculate your baseline and let us know on a voluntary short survey if your score improved significantly in any areas. Open the attachment

Step 2: Go to WebMD by clicking on the following link:
WebMD BMI Plus Calculator. The BMI Calculator is an innovative health calculator that gives personal results on 6 different weight and fitness measurements. Whether you’re just getting started or have already begun your journey, this calculator will help you understand your body’s needs as your body changes for optimal personal success! This information is also personal to you and is not shared with anyone else.
Step 3: Go to the Healthy Lifestyle Challenge 2011 web page by clicking on the following link:
Healthy Lifestyle Challenge Once there, click on the Weekly Tracking Sheet which can be found on the left side of the page. We recommend that you save this form to your desk top. It will then be easy to open the form each day and record your scores. At the end of each week, you will be asked to email or fax a copy of your weekly tracking sheet to us at:

Step 4: Each Week we will send a short weekly wellness communication. It is important that you take two minutes to read this. In addition to any challenge updates, we will highlight opportunities available in the community to support your efforts. We encourage you to take advantage of these opportunity to meet your fellow participants. These opportunities can also be used to accumulate bonus points! The weekly communication will also feature a wellness related link. By visiting the featured link, you will easily find the answer to a Bonus Question and an opportunity for another 5 points!
Step 5: For those of you who will use this opportunity to improve your BMI, you can earn a special Transformation bonus. More details can be found on the weekly tracking sheet. If you are already are within the recommended BMI, you can request another challenge which may be considered for the transformation bonus (i.e., smoking cessation, drinking, etc.).
How can I win prizes? There is a maximum score of 700 points (plus bonus points) in a week. Each week that you turn in your tracking sheet and meet a minimum goal of 200 points, you will earn one free raffle ticket. The top 10 scores each week will earn two free raffle tickets. The top score each week will earn one additional raffle ticket. All of your accumulated tickets will remain in the challenge basket from week to week. However, for any week a participant doesn’t turn in his/her tracking sheet, they will be ineligible for that week’s drawing.
No participant will be allowed to win a prize two weeks in a row. Everyone will be eligible for the final weeks drawing(s) regardless of any previous prizes earned.
