Please take the time to record your current height and weight.  Then click on the link to the BMI calculator.  Record this information on your personal Baseline Screening_Tool.  This information is for your personal use.  We will ask you at the end of weeks 5 and 10 to complete a short survey to help us gauge the success of this challenge.

The BMI Plus Calculator is an innovative health and weight calculator that gives personal results on 6 different weight and fitness measurements. Whether you're just getting started or have already begun your journey, this calculator will help you understand your body's needs as your body changes for optimal personal success.  You can simply use the WebMD Calculator to determine your BMI or you can use this tool to get a better understanding of your overall fitness.

  • BMI – Body Mass Index BMI is used to find out if a person is underweight, normal weight, overweight, or obese
  • Waist-to-Height Ratio Body shape (determined by Waist-to-Height Ratio) is often considered to be the best indicator of health related to weight.
  • RMR – Resting Metabolic Rate The number of calories you burn if you do nothing all day.
  • Healthy Body Weight Range The range with the least risk of weight-related health problems.
  • Daily Caloric Intake The recommended number of calories from food/drinks to meet your weight goals.
  • Target Heart Rate The heart rate needed to achieve maximum exercise results.

This web tool will assist you in developing an action plan to meet both your physical activity and nutrition goals. This tool does not provide medical advice.  

Updates:

Note that we are changing the eligibility requirement for weekly drawings.  As the rules state, every week that a participant submits their tracking sheet an earned raffle ticket will be placed in the challenge basket.  The top 10 scores each week will earn an additional raffle ticket and the top score will earn a third raffle ticket.  Initially the rules stated that, in the event that a participant failed to submit two qualifying tracking sheets, their tickets would be removed from the challenge basket. They would start over accumulating raffle tickets.

Provide your self assessment in each of the areas below with a rating of 0 for Poor to 15 for excellent. This is your own personal assessment, so feel free to assign any value that you believe is most closely aligned with your present status in each area. For example, you might place yourself between Good and Excellent for Physical Activity and assign yourself 13 points.

Poor:                      0 Points   (between 0 - 4)
Fair:                       5 Points   (between 5 - 9)

[2] Physical Health: comprises questions on topics a doctor may talk about, like Body Mass Index, daily energy, physical pain, history of illness, and medical conditions.
